Stephen P. Niemczyk, D.M.D.

Dr. Niemczyk graduated from the University Of Pennsylvania School Of Dental Medicine, receiving his D.M.D. in 1982 and his postgraduate Endodontic certificate in 1984. He was appointed Clinical Assistant Professor in the Department of Endodontics in 1984, and established a private practice limited to Endodontics in Drexel Hill, PA. He lectured extensively within the SDM at the undergraduate and postgraduate levels, directed the pre-clinical endodontic laboratory and was later named to the full time position of Director of Undergraduate Endodontics.

Dr. Niemczyk also served as the interim Post Graduate Director and Endodontic Clinic Director during his tenure at the SDM. He was the first director of the laboratory portion of the two-day Microscopic Endodontic Surgery Course, the first university-based course of its kind in the U.S. He was the recipient of many awards, including consecutive Earl Banks Hoyt Awards for Academic Teaching Excellence. Since 1995, Dr. Niemczyk has been the Director of Endodontic Microsurgery at Harvard University in Boston, MA and Albert Einstein Medical Center in Philadelphia. He also serves as a surgical consultant to the Postgraduate Endodontic Program at the Walter Reed National Medical Center (formerly the National Naval Medical Center) in Bethesda, MD., the U. S. Army Endodontic Residency Program in Fort Gordon, GA.

Dr. Niemczyk has lectured extensively in the U.S., Central America, Europe and Asia on topics ranging from Microsurgery and Intentional Replantation to advances in Non-surgical Endodontics and Cone-Beam CT use in Endodontics. He has authored numerous papers, a chapter on Microsurgery in Dental Clinics of North America, and is a member of the Scientific Advisory Board for the Journal of Endodontics and the Journal of Oral and Maxillofacial Surgery, Medicine and Pathology. He is the founder of Endodontic Microsurgical Innovations, dedicated to the development of new treatment techniques and instrumentation. He is a member of the Pennsylvania and New Jersey Dental Associations, American Association of Endodontists, Delta Sigma Delta Dental Fraternity and numerous study clubs, including the L.I. Grossman in Philadelphia and Edward C. Penick in Washington, D.C. (President, 2008-2009) He is a Diplomate of the American Board of Endodontics, and maintains a full time practice limited to Endodontics in Drexel Hill, PA.

Although all practitioners possess a basic endodontic skill set, the treatment environment has become more challenging and confusing, particularly with regard to diagnosis and canal debridement. Presenting signs and symptoms can be contradictory, complicating the treatment decision tree, and the myriad of instrumentation options can be daunting in terms of their sequences and applications. This all-day course addresses these concerns, as well as other procedural influences such as irrigation and canal location.

The morning session begins with the what, why and how of Diagnosis: what are the etiologies/symptoms, why do they present that way and how are they treated. Testing protocols and techniques will be discussed, and the participants will be familiarized with the AAE flow schedule for treatment and referral. Tooth-specific anatomy will be presented as a foundation for Anatomically Directed Access Preparation (ADAP), and these concepts will be reinforced in the hands-on portion of the course. The latest innovation in canal debridement using the Wave One Gold™ reciprocating file will be introduced and implemented for the instrumentation portion of the hands-on session. Proper technique and concepts will be stressed, with emphasis on obtaining and maintaining patency, the importance of irrigation and the prerequisites for filling the prepared root canal spaces.

Obturation of the prepared spaces will be accomplished using a Carrier-based system, GuttaCore™.  The technique will be explained in depth, highlighting successful placement strategies in typical and atypical anatomical spaces. The day concludes with an introduction to ultrasonics and its’ application in locating calcified canals orifices and uncovering auxiliary anatomy. Attendees are encouraged to bring their loupes with them for this portion of the course, and a limited number of Surgical Operating Microscopes will be available to demonstrate the effectiveness of the ultrasonic excavations at higher magnifications.

Course Objectives
At the conclusion, the participants should be able to:

  • Formulate a correct diagnosis and treatment plan for most patient scenarios.
  • Identify and be familiar with a unique instrumentation technique in a simplified sequence.
  • Understand the importance of proper irrigation and the steps to achieve it.
  • Utilize 3D fluid dynamics during thermoplastic obturation, and understand the steps required to achieve this using this method.
  • Appreciate the ease and efficiency afforded by increased magnification in conjunction with ultrasonics in the discovery and localization of canal anatomy.

Hands-on Workshop
This endodontic hands-on workshop is designed to reinforce and implement the techniques presented in the lecture. Participants will utilize WaveOne Gold™ shaping files to prepare root canals for 3-D obturation in extracted teeth (participants must bring 3-4 extracted teeth which have been previously accessed). Clinicians will have the opportunity to obturate their shaped canals with a carrier-based method into these fully-shaped root canals. Every station will be equipped with the equipment and supplies necessary to participate fully, and a Microscope/ ultrasonic station will be available for participants to refine their access and locate calcified anatomy.
